Joining HNA
Joining HNA is easy.
To get started as a Member of Healthcare Networks of America
Follow these steps:
- Step 1: Complete the downloadable Enrollment Form, and submit membership fees. You may FAX it to 602-485-3100 or mail in the forms. If you are a facility download Facility Enrollment with Agreement For
- Step 2: A confirmation will be sent to you upon completion with a full Health Plan List/Fee Schedule and registration instructions for our Members Resource Center. As part of the HNA membership ALL provider offices are asked to register on-line in the so that we may communicate updated payer contracts with you. Registration may be done for each email in your office.
